ABSTRACT

La sífilis congénita es causada por la infección del feto con Treponema pallidum durante el embarazo. Los síntomas son variables. Si bien es común el daño endotelial, no suele estar presente en los casos congénitos. Reportamos el caso de un lactante de 42 días de vida hospitalizado por masa abdominal. Las imágenes confirmaban la presencia de una lesión en el lóbulo hepático izquierdo sin efecto de masa. Las biopsias mostraron cambios compatibles con infarto y hepatitis neonatal. Las serologías del paciente y de su madre confirmaron el diagnóstico de sífilis congénita, y recibió tratamiento con penicilina intravenosa. El hígado se encuentra protegido de los daños isquémicos gracias a su doble irrigación, pero la acumulación de noxas puede haber provocado dicha presentación inusual. Tres meses más tarde, el paciente se encontraba libre de síntomas y la resonancia de control mostró atrofia del lóbulo izquierdo, mientras el resto del parénquima no presentaba alteraciones.


Congenital syphilis is caused by Treponema pallidum infection of the fetus during pregnancy. Symptoms are variable. While endothelial damage is common, it is not usually present in congenital cases. Here we report the case of a 42-day-old infant hospitalized due to an abdominal mass. Imaging studies confirmed the presence of an injury in the left lobe of the liver without mass effect. Biopsies showed changes compatible with infarction and neonatal hepatitis. The patient's and his mother's serologies confirmed the diagnosis of congenital syphilis, and he was treated with intravenous penicillin. The liver protected from ischemic injury by its double irrigation, but the accumulation of harmful agents may have caused this unusual presentation. Three months later, the patient was symptom-free, and the control MRI showed atrophy of the left lobe, while the rest of the parenchyma was unchanged.

ABSTRACT

La hepatitis por Treponema pallidum es una entidad poco frecuente y su diagnóstico representa un reto clínico. Treponema pallidum debe considerarse como etiología presuntiva en todo paciente con enfermedad hepática aguda, en el cual se hayan descartado otras causas más frecuentes. Se presenta el caso de un paciente joven, inmunocompetente, quien presentó elevación de los valores de las pruebas hepáticas con patrón colestásico y lesiones maculopapulares en palmas y plantas. Dado su cuadro clínico, las pruebas diagnósticas y la respuesta a la terapia antimicrobiana instaurada, se estableció el diagnóstico de colestasis por una sífilis secundario sifilítiao. Es importante incluir la sífilis secundaria entre las posibles causas de enfermedad hepática aguda.


Hepatitis due to Treponema pallidum is a rare entity and its diagnosis represents a clinical challenge. Treponema pallidum should be considered as a presumptive etiology in all patients with acute liver disease, when other frequent causes have been ruled out. We present the case of a young, immunocompetent patient with elevated values in his liver tests, a cholestatic pattern, and maculopapular lesions on his palms and soles. Given his clinical picture, diagnostic tests, and response to the antimicrobial therapy, a diagnosis of cholestasis due to secondary syphilis has been established. It is important to include secondary syphilis within the possible causes of acute liver disease.

Introduction: Congenital syphilis is a serious public health problem that causes high rates of intrauterine morbidity and mortality, revealing flaws and weaknesses in the health system. Objective: to report a case of congenital syphilis in a university hospital in the Center-South Region of the State of Rio de Janeiro, Brazil. Case report: A pregnant woman, aged between 19 and 23 years old, carrying a Pregnant Woman's Handbook with a record of seven prenatal consultations and a note of the serological reaction for positive syphilis, but without any treatment, hospitalized at the University Hospital of Vassouras (RJ), in labor, gave birth to a newborn (NB) with a clinical picture and serological test of congenital syphilis. The NB required care in an intensive care unit and was discharged 28 days after birth. Scraping of skin lesions of the NB and placenta was performed for analysis by molecular biology (PCR in house) and genetic material of Treponema pallidum was detected. Conclusion: Congenital syphilis is a serious outcome of syphilis during pregnancy, consuming high financial resources and significant emotional distress for the mother, father, the whole family, as well as for the health teams. Our case report was the first that we are aware of in Brazil with a diagnosis by PCR for positive Treponema pallidum of skin scraping and placental fragment. It also showed poor quality prenatal care, a common factor in most cases of CS in our reality

Introducción: La sífilis es una enfermedad infecciosa y sistémica producida por la bacteria Treponema pallidum, cuya principal vía de trasmisión son las relaciones sexuales desprotegidas. Objetivo: Caracterizar a pacientes reclusos con sífilis según variables clínicas y epidemiológicas. Métodos: Se realizó un estudio observacional, descriptivo y transversal, desde enero del 2016 hasta diciembre del 2020, de 66 hombres recluidos en el Centro Penitenciario Mar Verde de Santiago de Cuba, quienes fueron diagnosticados con sífilis. En el procesamiento estadístico de la información se aplicaron la prueba de probabilidades exactas de Fisher-Irving y la de independencia de la Χ2 para determinar si existía alguna asociación entre las variables cualitativas. Resultados: En la serie primó el diagnóstico de sífilis latente (100,0 %), así como el grupo etario de 25 a 34 años (33,3 %) y el nivel escolar de secundaria básica (77,3 %). La mayoría de los internos no tenían antecedente alguno de enfermedades de transmisión sexual (90,0 %), habían contraído la infección bacteriana en el interior del penal (74,2 %), eran multirreincidentes (45,5 %) y no disfrutaban de visitas conyugales (74,2 %). Conclusiones: Al igual que en otros países, en Cuba la sífilis resulta un importante problema de salud en los centros penitenciarios, donde la implementación de un riguroso control de las infecciones de transmisión sexual podría contribuir a reducir su incidencia.


Introduction: The syphilis is an infectious and systemic disease caused by the Treponema pallidum bacteria whose main way of transmission is the unprotected sexual intercourse. Objective: To characterize prisoner patients with syphilis according to clinical and epidemiological variables. Methods: An observational, descriptive and cross-sectional study was carried out from January, 2016 to December, 2020, of 66 men confined in Mar Verde Penitentiary Center in Santiago de Cuba who were diagnosed with syphilis. In statistical data processing, Fisher-Irving's exact test and chi-squared test of independence were applied to evaluate the association between the qualitative variables. Results: In the series there was a prevalence of the diagnosis of latent syphilis (100.0 %), as well as the 25 to 34 age group (33.3 %) and the basic secondary school level (77.3 %). Most of the prisoners didn't have history of sexually transmitted diseases (90.0 %), they got infected inside the prison (74.2 %), were multirreoffenders (45.5 %) and didn't enjoy marital relationships (74.2 %). Conclusions: In Cuba syphilis is an important health problem in the penitentiary centers as in other countries, where a rigorous control of sexually transmitted diseases could contribute to reduce its incidence.

ABSTRACT

Objetivo: Describir tres situaciones clínicas en las que se presentan distintas manifestaciones bucales para una misma entidad patológica. En los tres casos la sospe- cha diagnóstica de sífilis primaria se basó en la presencia de una adenopatía. Los estudios de laboratorio permitieron confirmar el diagnóstico de sífilis. Por su polimorfismo y variabilidad clínica en sus diferentes etapas evolutivas, la sífilis es descripta clásicamente como "la gran simuladora". Este artículo propone que la presencia de adenomegalias características puede ser una clave para orientar el diagnós- tico de la patología, lo cual no ha sido aún reportado en la literatura. Casos clínicos: Se presentan tres casos clínicos de pa- cientes que acudieron a una consulta estomatológica privada y al Servicio de Estomatología del Hospital Alemán de Bue- nos Aires. Los tres acuden con signos y síntomas diferentes, pero compartiendo la presencia de adenopatías múltiples, en las que se destaca un elemento ganglionar más voluminoso (AU)


Aim: To describe three clinical cases that present dif- ferent oral manifestations for the same pathological entity. In all three cases, the suspected diagnosis of primary syph- ilis was based on the presence of an adenopathy. Labora- tory studies confirmed the diagnosis of syphilis. Due to its polymorphism and clinical variability in the different evo- lutionary stages, syphilis is classically described as "the great simulator". This article proposes that the presence of characteristic adenomegalies can be a key to guide the di- agnosis, which has not yet been reported in the literature. Clinical reports: Three clinical cases of patients who attended a private stomatology consultation and the Stoma- tology Service of the Hospital Alemán de Buenos Aires are presented. The three patients showed different signs and symptoms but shared the presence of multiple adenopathies with a more voluminous ganglial element (AU)

Resumen Se realizó un estudio descriptivo observacional transversal en los municipios de Posadas y Garupá con el objetivo de describir la situación social, el conocimiento sobre infecciones de transmisión sexual (ITS) y la prevalencia de infección por Treponema pallidum y por el virus de inmunodeficiencia humana (HIV) en adolescentes escolarizados en área vulnerables del Departamento Capital de Misiones (Argentina). Se realizaron 1336 encuestas y 347 pruebas serológicas a adolescentes de 19 establecimientos de nivel primario y secundario, el 64% (857) fueron de Posadas y el 36% (479) de Garupá. La mediana de la edad fue de 13 años. Las respuestas afirmativas sobre la información suministrada en salud reproductiva fue en Posadas 73% y en Garupá 49%; el ámbito de información citado con mayor frecuencia fueron las escuelas (Posadas 92% y Garupá 76%). La mayoría respondió que en su grupo de amigos se ingerían bebidas con alcohol (Posadas 64% y Garupá 52%) y, con menor frecuencia, drogas (Posadas 21% y Garupá 17%). Las respuestas correctas sobre conocimientos de ITS fueron superiores al 82%. Más del 80% residían en casas de material con baño instalado y el 9% en condiciones de hacinamiento. No se identificaron adolescentes escolarizados con infección por HIV, pero sí un caso con infección por T. pallidum (prevalencia: 0,29%; intervalo de confianza del 95%: 0,05-1,62%). El estudio mostró muy baja prevalencia de infección por T. pallidum y nula para HIV en adolescentes escolarizados residentes en áreas socialmente vulnerables. La constitución familiar y la escolarización podrían constituir factores de protección para estas infecciones.


Abstract A cross-sectional observational descriptive study was carried out in the municipalities of Posadas and Garupá with the aim of describing the social situation, knowledge about sexually transmitted infections (STIs) and the prevalence of Treponema pallidum and human immunodeficiency virus (HIV) infection in adolescents enrolled in vulnerable areas of the Capital Department of Misiones (Argentina). A total of 1,336 surveys and 347 serological tests were carried out on adolescents from 19 primary and secondary schools, 64% (857) were from Posadas and 36% (479) from Garupá. The median age was 13 years. The affirmative answers about the information provided in reproductive health were 73% in Posadas and 49%; in Garupá; the area of information most frequently cited was schools (Posadas 92% and Garupá 76%). Most responded that their group of friends drank alcoholic beverages (Posadas 64% and Garupá 52%) and, less frequently, drugs (Posadas 21% and Garupá 17%). The correct answers about knowledge of STIs were higher than 82%. More than 80% resided in brick houses with installed bathrooms and 9% in overcrowded conditions. School adolescents with HIV infection were not identified, but one case with T. pallidum infection was detected (prevalence: 0.29%; 95% confidence interval: 0.05-1.62%). The study showed a very low prevalence of T. pallidum infection and zero prevalence for HIV in school-going adolescents residing in socially vulnerable areas. Family constitution and schooling could be protective factors for these infections.


Resumo Foi realizado um estudo descritivo observacional transversal nos municípios de Posadas e Garupá com o objetivo de descrever a situação social, o conhecimento sobre infecções sexualmente transmissíveis (ISTs) e a prevalência da infecção pelo Treponema pallidum e pelo vírus da imunodeficiência humana (HIV) em adolescentes matriculados em áreas vulneráveis do Departamento Capital de Misiones (Argentina). Foram realizadas 1.336 inquéritos e 347 testes sorológicos em adolescentes de 19 escolas de ensino fundamental e médio, sendo 64% (857) de Posadas e 36% (479) de Garupá. A mediana de idade foi de 13 anos. As respostas afirmativas sobre as informações prestadas em saúde reprodutiva foram em Posadas 73% e em Garupá 49%; a área de informação mais citada foi as escolas (Posadas 92% e Garupá 76%). A maioria respondeu que seu grupo de amigos consumia bebidas alcoólicas (Posadas 64% e Garupá 52%) e, com menor frequência, drogas (Posadas 21% e Garupá 17%). As respostas corretas sobre o conhecimento de ISTs foram superiores a 82%. Mais de 80% residiam em casas materiais com banheiros instalados e 9% em condições de superlotação. Adolescentes escolares com infecção pelo HIV não foram identificados, mas foi identificado um caso com infecção por T. pallidum (prevalência: 0,29%; intervalo de confiança de 95%: 0,05-1,62%). O estudo mostrou prevalência muito baixa de infecção por T. pallidum e prevalência zero para HIV em adolescentes escolares residentes em áreas de vulnerabilidade social. A constituição familiar e a escolaridade podem ser fatores protetores para essas infecções.

ABSTRACT

Research to identify a syphilis vaccine began shortly after the isolation of the first Treponema pallidum subspecies pallidum (T. pallidum) strain in 1912 by Nichols and Hough and the identification of several possible animal models for the infection, with the rabbit being the best one. During the century following T. pallidum isolation, none of the numerous immunization/challenge experiments performed with preparations ranging from whole-inactivated T. pallidum cells to recombinant proteins yielded an effective vaccine, and the search for a vaccine languished. Recently, however, scientific communities have experienced a resurgence in interest in developing a syphilis vaccine due to 1. the awareness that syphilis constitutes a tremendous burden for maternal health, particularly in low- and middle-income nations; 2. the improved understanding of the immunological processes leading to pathogen clearance during natural infection and of the mechanisms this pathogen developed to persist in the host; 3. the availability of a near-complete list of T. pallidum genes encoding putative surface-exposed antigens, which represent the most likely vaccine candidates; and, last but not least, 4. the effort made to expand the knowledge on the genetic and antigenic diversity of these vaccine candidates in strains circulating worldwide. Thus far, the most recent vaccine designs based on a subset of the pathogen's surface-exposed antigens have provided immunized rabbits with a significant but incomplete protection upon infectious challenge. Nonetheless, the outcomes of these experiments help investigators refine strategies to achieve a formulation with the highest chances of moving from preclinical experimental settings to clinical trials. This editorial focuses on a subset of the strategies currently believed to be essential for vaccine development, namely, the improvement of our still limited understanding of the genomic diversity in T. pallidum strains from diverse geographical locations through the collection and isolation of modern syphilis strains and the identification of protective epitopes in potential vaccine targets by evaluating the ability of monoclonal antibodies to bind the target antigen and facilitate pathogen clearance. The use of genetic engineering of the syphilis spirochete to identify target surface proteins with an essential or near-essential role in T. pallidum biology to target in immunization/challenge experiments is also discussed.

ABSTRACT

Objetivo: O objetivo desta pesquisa foi estimar a prevalência da positividade de testes sorológicos para sífilis nos pacientes atendidos pelo Hospital Universitário do Oeste do Paraná (HUOP) e descrever parte de suas características socioepidemiológicas. Métodos: Estudo transversal que utilizou como fonte de dados informações do prontuário eletrônico Tasy®, referentes aos resultados de exames de VDRL e FTA-Abs realizados no HUOP, entre janeiro de 2015 e dezembro de 2019. Resultados: Durante este período foram liberados laudos de 26.968 testes de VDRL, realizados no Laboratório do HUOP e o nú- mero de resultados reagentes foi de 1.014. Em relação ao FTA-Abs, foram liberados 383 resultados e destes 116 foram reagentes. O estudo evidenciou um aumento de resultados reagentes do teste não treponêmico VDRL de 2017 a 2019. Conclusão: Neste estudo foi observado um aumento progressivo no diagnóstico de sífilis, e isto reflete a situação da doença em todo o Brasil que, mesmo com protocolos de diagnóstico e tratamento, persiste numa posição de grande destaque na saúde pública. Os dados apresentados demonstram a importância do serviço do HUOP para o diagnóstico desta doença para grande parte do oeste paranaense.


Objective: The objective of this research was to estimate the positivity prevalence of syphilis serological tests, in the patients attended in the State Universitary Hospital from Western of Paraná (HUOP), and to describe part of their socioepidemiological characteristics. Methods: Transversal study, which made use of the informations contained in the electronic medical reports software Tasy®, regarding the results of exams of VDRL and FTA-Abs, performed in the HUOP between january of 2015 and december of 2019. Results: During the studied period, reports on 26,968 VDRL tests were performed in the HUOP laboratory and the number of reagent results is 1,104. Towards the FTA-Abs test, 383 tests were performed and 116 of these were positive. The study showed an increasing number of reagent results for VDRL between 2017 and 2019. Conclusion: In this study, a progressive increase in the diagnosis of syphilis was observed, this reflects the situation of the disease all around the brazillian country, in which, even with well defined diagnostic and treating protocols, the infection persists in a prominent position for the public health. The presented data shows the importance of the HUOP's service to the population, in regards to the syphilis diagnose.

Resumen La sífilis es una enfermedad de transmisión sexual causada por el Treponema pallidum. Sin tratamiento es crónica y altamente contagiosa. Sus variadas manifestaciones clínicas dificultan su diagnóstico. Presentamos el caso de una lesión sifilítica en cavidad oral con alta sugerencia clínica de carcinoma de células escamosas. Caso clínico: hombre de 31 años que presenta gran úlcera en mucosa labial inferior derecha de bordes indurados, 2 meses de evolución, asociada a dos adenopatías en cuello. Se realiza una minuciosa anamnesis y exámenes de laboratorio, confirmando el diagnóstico de una infección treponémica. Se le brindó tratamiento y la lesión resolvió. Los títulos de seguimiento fueron no reactivos. Conclusiones: Se demuestra la importancia de mantener sospecha de infección sifilítica cuando un paciente presenta linfoadenopatía y una lesión oral, fundamentar un diagnóstico correcto proporciona una pronta atención y recuperación del paciente, evitando complicaciones que afecten su calidad de vida y a sus parejas sexuales.


Resumo A sífilis é uma doença sexualmente transmissível causada por Treponema pallidum. Sem tratamento, é crônico e altamente contagioso. Suas variadas manifestações clínicas dificultam seu diagnóstico. Apresentamos o caso de uma lesão sifilítica na cavidade oral com uma alta sugestão clínica de carcinoma de células escamosas. Caso clínico: homem de 31 anos de idade, com úlcera grande na mucosa do lábio inferior direito, com bordas endurecidas, 2 meses de evolução, associado a linfadenopatia do pescoço. Uma anamnese completa e exames laboratoriais são realizados, confirmando o diagnóstico de uma infecção treponêmica. O tratamento foi realizado e a lesão resolvida. Os títulos de acompanhamento não foram reativos. Conclusões: A importância de manter uma suspeita de infecção sifilítica quando um paciente apresenta linfadenopatia e uma lesão oral é demonstrada, justificando um diagnóstico correto, proporcionam atenção e recuperação imediatas do paciente, evitando complicações que afetam sua qualidade de vida e seus parceiros sexuais.


Abstract Syphilis is a sexually transmitted disease caused by Treponema pallidum. Without treatment, it is chronic and highly contagious. Its varied clinical manifestations make it challenging to diagnose. We present the case of a syphilis oral lesion with a strong clinical suggestion of squamous cell carcinoma. Clinical case: a 31-year-old man with a large ulcer in the right lower lip mucosa with indurated edges, two months of evolution, associated with two enlarged neck lymph nodes (lymphadenopathy). A thorough history is taken, and laboratory tests are run, confirming the treponemal infection diagnosis. He was treated, and the injury resolved. Follow-up tests were nonreactive. Conclusions . It is important to suspect a syphilitic infection when a patient has lymphadenopathy and an oral lesion. Making the correct diagnosis allows for the proper care and recovery, avoiding complications that affect the patient's quality of life and sexual partners.

ABSTRACT Background: Large-scale epidemiological studies of seroprevalence of antibodies against SARS-CoV-2 often rely on point-of-care tests that provide immediate results to participants. Yet, little is known on how long rapid tests remain positive after the COVID-19 episode, or how much variability exists across different brands and even among batches of the same test. Methods: In November 2020, we assessed the sensitivity of three tests applied to 133 individuals with a previous positive PCR result between April and October. All subjects provided finger prick blood samples for two batches (A and B) of the Wondfo lateral-flow IgG/IgM test, and dried blood spot samples for the S-UFRJ ELISA test. Results: Overall sensitivity levels were 92.5% (95% CI 86.6-96.3), 63.2% (95% CI 54.4-71.4) and 33.8% (95% CI 25.9-42.5) for the S-UFRJ test, Wondfo A and Wondfo B tests, respectively. There was no evidence of a decline in the positivity of S-UFRJ with time since the diagnosis, but the two Wondfo batches showed sharp reductions to as low as 41.9% and 19.4%, respectively, for subjects with a positive PCR in June or earlier. Positive results for batch B of the rapid test were 35% to 54% lower than for batch A at any given month of diagnosis. Interpretation: Whereas the ELISA test showed high sensitivity and stability of results over the five months of the study, both batches of the rapid test showed substantial declines, with one of the batches consistently showing lower sensitivity levels than the other. ELISA tests based on dried-blood spots are an inexpensive alternative to rapid lateral-flow tests in large-scale epidemiological studies.

ABSTRACT

ABSTRACT Homeless persons have been considered as one of the most susceptible populations to sexually transmitted infections. In Brazil, these population experienced an increase of 140% from 2012 to 2020. Accordingly, the present study aimed to assess the seroprevalence of anti-Treponema pallidum, anti-HCV, anti-HIV antibodies, and the risk factors associated with homeless persons in a daytime attendance shelter of São Paulo city during the syphilis epidemic in Brazil. Blood samples of 116 volunteers and epidemiological data were conveniently collected in the shelter from June through August 2018. Detection of syphilis, HCV, and HIV antibodies was performed by chemiluminescent microparticle immunoassay (CMIA). CMIA-reagent samples for anti-T. pallidum antibodies were confirmed by Venereal Disease Research Laboratory (VDRL) non-treponemal test. VDRL non-reagent samples were confirmed by treponemal rapid immunochromatographic test. A rapid immunoblot assay confirmed seropositivity to HIV. Overall, anti-T. pallidum antibodies were observed in 29/116 (25.0%), anti-HCV antibodies in 4/116 (3.4%), and anti-HIV antibodies in 2/116 (1.7%) individuals, both co-infected with anti-T. pallidum antibodies. Associated risk factors for syphilis in homeless persons were being born or previously living in another city (p = 0.043) and becoming homeless due to family conflicts (p = 0.035). Besides homeless vulnerability, worldwide shortage of benzathine penicillin supply and increasing of syphilis testing access through rapid testing in primary health care services may have also impacted disease spreading at the time. The prevalence of syphilis found herein is the highest worldwide to date in this population.

ABSTRACT

Introdução: A sífilis durante a gravidez é um grave problema de saúde pública. Em 2016, foram notificados 37.436 casos de sífilis em gestantes e 20.474 casos de sífilis congênita, com 185 óbitos no Brasil. O fator determinante para a diminuição dos casos é o pré-natal de qualidade. Objetivo: Avaliar o perfil dos casos notificados de sífilis em gestante no estado de Minas Gerais, entre 2013 a 2017. Métodos: Trata-se de um estudo transversal descritivo sobre o perfil epidemiológico dos casos notificados de sífilis em gestante em Minas Gerais entre 2013 a 2017. Os dados foram obtidos no Sistema de Informação de Agravos de Notificação, a partir do Departamento de Informática do Sistema Único de Saúde e categorizados quanto ao número de casos, ano de diagnóstico, faixa etária, nível de escolaridade, raça, região e estágio clínico ao diagnóstico. Resultados: Em 2017 foi demonstrado um aumento de 132% dos casos de gestantes com sífilis, em comparação com 2013. Houve predomínio na região central do estado (43,30%). 92,50% se referem a gestantes moradoras da zona urbana. A faixa etária mais acometida foi 20 a 39 anos. A maior parte (46,3%) se declara como parda. E a escolaridade 5ª a 8ª série incompleto do ensino fundamental foi a mais prevalente (16,05%). A classificação clínica mais comum da sífilis materna foi a primária (34,7%). Conclusão: A assistência pré-natal adequada deve ser iniciada precocemente. É necessária a capacitação das equipes das Unidades Básicas para garantirem ações educativas, identificação de situações de risco, diagnóstico e tratamento oportuno.


Introduction: Syphilis during pregnancy is a serious public health problem. In 2016, 37,436 cases of syphilis in pregnant women and 20,474 cases of congenital syphilis were reported, with 185 deaths in Brazil. The determining factor for the decrease in cases is the prenatal quality. Objective: To evaluate the profile of notified cases of syphilis in pregnant women in Minas Gerais, between 2013 to 2017. Methods: This is a cross-sectional descriptive study on the epidemiological profile of notified cases of syphilis in pregnant women in Minas Gerais from 2013 to 2017. Data were obtained from the Notification of Disease Information System, the from the Department of Informatics of the Unified Health System and categorized according to the number of cases, year of diagnosis, age group, level of education, race, region and clinical stage at diagnosis. Results: In 2017 there was a 132% increase in cases of pregnant women with syphilis compared to 2013. There was a predominance in the central region of the state (43.30%). 92.50% refer to pregnant women living in urban areas. The most affected age group was 20 to 39 years. Most (46.3%) declare themselves as brown. And the 5th to 8th grade incomplete elementary schooling was the most prevalent (16.05%). The most common clinical classification of maternal syphilis was primary (34.7%). Conclusion: Appropriate prenatal care should be started as early as possible. The training of Basic Units teams is necessary to ensure educational actions, identification of risk situations, diagnosis and timely treatment.

ABSTRACT

Os temas sífilis congênita e criança exposta à sífilis compõem o Protocolo Clínico e Diretrizes Terapêuticas para Atenção Integral às Pessoas com Infecções Sexualmente Transmissíveis, publicado pelo Ministério da Saúde do Brasil em 2020. Tal documento foi elaborado com base em evidências científicas e validado em discussões com especialistas. Este artigo apresenta orientações para o manejo clínico da sífilis em gestantes e da sífilis congênita, enfatizando a prevenção da transmissão vertical do Treponema pallidum. Nele estão contemplados aspectos epidemiológicos e clínicos desses agravos, bem como recomendações aos gestores no manejo programático e operacional da sífilis. Também se incluem orientações para os profissionais de saúde na triagem, diagnóstico e tratamento das pessoas com infecções sexualmente transmissíveis e suas parcerias sexuais, além de estratégias para ações de vigilância, prevenção e controle da doença.


The topics of congenital syphilis and children exposed to syphilis are part of the Clinical Protocol and Therapeutic Guidelines for Comprehensive Care for People with Sexually Transmitted Infections, published by the Brazilian Ministry of Health in 2020. The Protocol and Guidelines have been developed based on scientific evidence and validated in discussions with specialists. This article provides guidelines for clinical management of both syphilis in pregnant women and also congenital syphilis, emphasizing prevention of vertical transmission of Treponema pallidum. Epidemiological and clinical aspects of these infections are presented, as well as recommendations for health service managers regarding the programmatic and operational management of syphilis. The article also includes guidelines for health professionals on screening, diagnosing and treating people with sexually transmitted infections and their sex partners, in addition to strategies for syphilis surveillance, prevention and control actions.


Los temas sífilis congénita y niños expuestos a la sífilis componen el Protocolo Clínico y Directrices Terapéuticas para la Atención Integral a Personas con Infecciones de Transmisión Sexual, publicado por el Ministerio de Salud de Brasil en 2020. Tal documento fue elaborado con base en evidencia científica y validado en discusiones con especialistas. Este artículo presenta directrices para el manejo clínico de la sífilis en mujeres embarazadas y de la sífilis congénita, con énfasis en la prevención de la transmisión vertical del Treponema pallidum. Se contemplan aspectos epidemiológicos y clínicos de la infección, así como recomendaciones para gestores en la gestión programática y operativa de la sífilis. También se incluyen directrices para profesionales de la salud en la detección, diagnóstico y tratamiento de personas con infecciones de transmisión sexual y sus parejas sexuales, además de estrategias para acciones de vigilancia, prevención y control de la enfermedad.

Objective@#The aim of the present study was to evaluate the performance of the simultaneous detection of HIV-1 RNA, HIV-1 DNA, and HCV RNA using one dried blood spot (DBS) as an alternative sample to plasma.@*Method@#A total of 571 paired DBS/plasma samples were collected from men who have sex with men (MSM) and injection drug users (IDUs), and serological and molecular assays were performed. Using plasma results as the reference standard, the performance of DBS tests for HIV-1 RNA, HIV-1 DNA, and HCV RNA was evaluated. Pearson's correlation coefficients and Bland-Altman analysis were performed to assess the correlation and concordance between DBS and plasma.@*Results@#Among paired plasma/DBS samples with detectable HIV-1 RNA and HCV RNA, five samples (5/32) were not detectable in DBS, while measurable HIV-1 RNA levels were present in plasma (1.44 to 3.99 log @*Conclusion@#The performance of the simultaneous detection of HIV-1 RNA, HIV-1 DNA, and HCV RNA using one DBS was acceptable. DBS, as an alternative sample to plasma, may be a viable option for the simultaneous detection of HIV-1 RNA, HIV-1 DNA, and HCV RNA in resource-limited settings or for individuals living in areas that are difficult to access.

RESUMEN Introducción: La neurosífilis es una enfermedad infecciosa crónica ocasionada por Treponema pallidum y puede producir una gran variedad de signos y síntomas neuropsiquiátricos, lo que complica su diagnóstico. Descripción del caso: Se presenta el caso de un paciente de 40 años que consultó al servicio de urgencias por una convulsión de novo, junto con un cuadro crónico de deterioro cognitivo y psicosis. Se le realizaron los estudios pertinentes para el diagnóstico de neurosífilis y se inició el tratamiento recomendado. El paciente presentó mejoría clínica y fue dado de alta. Discusión: Con resultados de serología VDRL positivos y hallazgos imagenológicos de atrofia cortical marcada, se consideró una neurosífilis parenquimatosa de tipo tardío, junto con franco deterioro cognoscitivo y psicosis. Se le dejó tratamiento con penicilina cristalina, que disminuyó la intensidad de los síntomas del paciente; sin embargo, el poco interés de este en asistir a sus controles disminuye sus probabilidades de una recuperación adecuada. Conclusiones: La neurosífilis se debe sospechar en pacientes con síntomas neurológicos o psiquiátricos clínicamente evidentes. El análisis de la serología de VDRL y los estudios de neuroimagen son importantes como evaluación inicial del paciente, que debe complementarse con pruebas cognitivas o examen mental para determinar el estado de deterioro cognitivo.


ABSTRACT Introduction: Neurosyphilis is a chronic infectious disease caused by Treponema pallidum that can cause a great variety of neuropsychiatric signs and symptoms, which complicates its diagnosis. Case description: This case occurred in a 40-year-old man who consulted the Emergency Department because of a convulsion (de novo) which was related to a chronic condition of cognitive impairment and psychosis. The appropriate studies were performed for the diagnosis of neurosyphilis and the recommended treatment was initiated. The patient presented clinical improvement and was discharged. Discussion: Positive VDRL serology results and imaging findings of marked cortical atrophy conducted to consider a late-stage parenchymatous neurosyphilis with serious cognitive impairment and associated psychosis. Treatment with crystalline penicillin was formulated, which reduced the intensity of the patient's symptoms; however, the patient's lack of interest to attend the check-ups significantly reduces his chances of an adequate recovery. Conclusions: Neurosyphilis must be suspected in patients with clinically evident neurological or psychiatric symptoms. Analysis of VDRL serology and neuroimaging studies are important as an initial evaluation of the patient and must be complemented with cognitive tests or mental examination to determine the state of cognitive impairment.

La coinfección entre el Treponema pallidum y el virus de la inmunodeiciencia humana (VIH) altera el curso clínico clásico de la sífilis aumentando la probabilidad de aparición de formas atípicas del secundarismo sifilítico. Entre estas formas se ha descripto a la sífilis elegante, entidad caracterizada por un exantema maculopapuloso descamativo, de aspecto anular, por lo general, con indemnidad de las regiones palmo plantar y de las mucosas. Se presenta un caso de sífilis secundaria, con lesiones típicas por su aspecto y localización, de sifílides elegantes en una paciente con diagnóstico de sida


Co-infection between Treponema pallidum and HIV alters the classic clinical course of syphilis, increasing the likelihood of atypical forms of syphilitic secondaryism. Among these forms, elegant syphilis has been described, an entity characterized by a desquamating maculopapular rash of annular appearance, with indemnity of the palmoplantar surface and mucous regions. Here, we present a case of secondary syphilis with typical lesions of elegant syphillides, in a patient diagnosed with AIDS

Resumen En el mundo, se ha evidenciado un aumento de los casos de sífilis, de sífilis gestacional y de sífilis congénita. Se presenta el caso de un recién nacido con sífilis congénita, hijo de una madre con sífilis latente de duración indeterminada (VDRL 1:4 diluciones) diagnosticada y tratada a la semana 12 de gestación, VIH negativa, con compañero seronegativo para sífilis; a pesar del tratamiento con tres dosis de 2'400 000 U de penicilina benzatínica, no modificó los títulos del VDRL ni en el control de la semana 25 de gestación, ni en el momento del parto. En el posparto, la madre fue diagnosticada con neurosífilis y recibió tratamiento con penicilina cristalina durante 14 días. El recién nacido fue diagnosticado con sífilis congénita por presentar VDRL 1:4 diluciones, aumento de aminotransferasas, hematuria y proteinuria, recibiendo tratamiento con penicilina cristalina durante 10 días. La paciente evolucionó favorablemente y el control a los 6 meses fue normal. Deben fomentarse medidas útiles en la prevención de la sífilis gestacional: métodos de barrera, conocimiento de la enfermedad y asistencia a control prenatal. Es fundamental identificar y tratar a las gestantes con sífilis mediante tamizaje serológico para prevenir la sífilis congénita; el seguimiento serológico debe ser estricto para verificar la eficacia del tratamiento e investigar las gestantes que no modifican los títulos después del tratamiento. Abstract In the world, there has been an increase in cases of syphilis, gestational syphilis and congenital syphilis. It's presented the case of a newborn with congenital syphilis, son of a mother with latent syphilis of indeterminate duration (VDRL 1: 4 dilutions) diagnosed and treated at week 12 of gestation, HIV negative, with seronegative partner for syphilis; despite treatment with three doses of 2'400 000 U of benzathine penicillin, the VDRL titres remained unaltered on both the control of the 25th week of gestation and at the time of delivery. The mother, in the postpartum period, was diagnosed with neurosyphilis and was treated with crystalline penicillin for 14 days. The newborn was diagnosed with congenital syphilis by presenting 1:4 VDRL dilutions, increased aminotransferases, hematuria and proteinuria; he was treated with crystalline penicillin for 10 days. Useful measures should be promoted in the prevention of gestational syphilis such as barrier methods, knowledge of the disease and assistance to prenatal control. It is mandatory to identify and treat pregnant women with syphilis by serological screening for the disease in order to prevent congenital syphilis. Serological follow-up should be strict to verify the effectiveness of the treatment and to investigate pregnant women who do not modify the titres after treatment. MÉD.UIS.2020;33(1):73-80.

ABSTRACT

Objetivo: estandarizar una qPCR para la determinación de T. pallidum en muestras de suero de pacientes con sífilis gestacional y congénita. Materiales y métodos: se optimizó una qPCR con sonda para la amplificación del gen TpN47 en muestras de suero, se evaluó la sensibilidad, especificidad y eficiencia analítica de la técnica. Se comparó con pruebas serológicas (VDRL y TPPA) y se calculó índice de concordancia Kappa. Resultados: la qPCR mostró un límite de detección de 0.113 femtogramos, especificidad analítica del 100% y fidelidad de 104%. Se evidenció correlación optima en la prueba, sugerida por un r2 de 0.99 y un valor p <0,0001 de la qPCR. Se observó acuerdo entre las pruebas serológicas y moleculares. Conclusiones: se desarrolló una herramienta molecular prometedora con buena sensibilidad, óptima especificidad analítica y gran potencial diagnóstico para la detección y hallazgo de T. pallidum subsp. pallidum, a través de la amplificación del gen TpN47 en muestras clínicas de pacientes con diagnóstico presuntivo de sífilis gestacional y congénita.


Objective: to evaluate a qPCR to detect T.pallidum in serum samples from patients with gestational and congenital syphilis. Methodology: qPCR with probe was optimized for the amplification of the TpN47 gene in serum samples, the sensitivity, specificity and analytical efficiency of the technique were evaluated. It was compared with the serological tests (VDRL and TPPA) and the Kappa concordance index was calculated. Results: the qPCR showed a detection limit of 0.113 femtograms, an analytical specificity of 100% and an accuracy of 104%. Optimal correlation was evidenced in the test, suggested by an r2 of 0.99 and a p value <0.0001 of the qPCR. An agreement was observed between serological and molecular tests. Conclusion: a promising molecular tool was developed with good sensitivity, excellent analytical specificity and great diagnostic potential for the detection and finding of T. pallidum subsp. pallidum, through the amplification of the TpN47 gene in serum from patients with gestational and congenital syphilis.

ABSTRACT

Resumo Apresentamos um caso de neurosífilis em um homem jovem, com queixa de baixa acuidade visual (BAV) em olho esquerdo. Cursou com lesões eritemato-descamativas nas palmas das mãos, plantas dos pés e úlceras orais, sem lesões genitais. O exame oftalmológico revelou arterite em arcada nasal superior no olho afetado. Apresentou VDRL (1:4096) e FTA-Abs positivos. O exame do líquor cefalorraquidiano foi negativo. O tratamento foi realizado com ceftriaxona 2g/ dia por 14 dias, associado à prednisona 0,5mg/kg oral 48h após início do antibiótico. Após 15 dias de tratamento, houve melhora da AV, regressão da vasculite e redução da titulação do VDRL para 1:128.


Abstract We present a case of neurosyphilis in a young man with a complaint of low visual acuity in the left eye. He had erythematous-scaly lesions on the palms of the hands, soles of the feet and oral ulcers, without genital lesions. The ophthalmic examination revealed arteritis in the upper nasal arcade in the affected eye. He presented VDRL (1: 4096) and FTA-Abs positive. The cerebrospinal fluid cerebrospinal fluid test was negative. The treatment was performed with ceftriaxone 2g / day for 14 days, associated with prednisone 0.5mg / kg oral 48h after antibiotic onset. After 15 days of treatment, there was improvement of AV, regression of vasculitis and reduction of VDRL titration to 1: 128.

Objetivo: Apesar de o tratamento ser de fácil acesso e também eficaz, a sífilis permanece em nosso meio e por isso foi realizado um levantamento de dados sobre os casos de sífilis em pacientes do Sistema Único de Saúde (SUS) atendidos no Laboratório de Análises Clínicas da PUC-GO (LAC) no período de janeiro a dezembro de 2018. Métodos: Trata-se de um estudo quantitativo e retrospectivo. Resultados: Foi constatado um aumento de 3,6% do número total de casos em relação ao ano de 2014 e um maior índice de prevalência no sexo masculino (16,6%). Além disso, verificou-se que 51,1% dos casos de VDRL reagente do LAC- PUC em 2018 eram de pacientes em idade fértil com faixa etária entre 21 e 40 anos. Conclusão: Isso demonstra a necessidade de uma mudança mais profunda, não somente nas políticas de Saúde Pública, mas também na conscientização social sobre os riscos da mesma.


Abstract Objective: Although the treatment is easy to access and also effective, syphilis remains in our country, so a data collection was carried out on syphilis cases in patients from the Unified Health System (SUS) attended at the Laboratory of Clinical Analysis of PUC-GO (LAC) from January to December 2018. Methods: This is a quantitative and retrospective study. Results: There was a 3.6% increase in the number of cases in relation to 2014 and a higher prevalence rate in males (16.6%). In addition, it was found that 51.1% of the cases of reactive VLDL of the LAC-PUC in 2018 are of patients of fertile age, with age between 21 and 40 years. Conclusion: This demonstrates the need for a more profound change, not only in Public Health policies, but also in social awareness about the risks of the same.
